Frequently Asked Questions

WHAT IS MED VOICE?

Our office uses a system called "Med Voice" to contact patients with appointment reminders and test results. If you receive an automated call, please call the information number (480) 423-7135 to retrieve your message. Your ID number is the first nine digits of your phone number if you do not choose to provide another number. All results are communicated to you either by the Med Voice system, in person during an appointment, or by a phone call. Keep in mind that some test results (certain labs) can take up to 2 weeks. You will not be contacted until all results are in. If you have not heard from us after 2 weeks, please contact us and we will check on the status at that time.  

HOW DO PHONE CALLS WORK?

Most incoming calls go to a voicemail while we are attending to patients in the office. Please leave a message that includes a phone number where you can be reached after 1 PM. If we can't reach you at the number you leave, we will leave a message for you and you will need to call again. Calls received before 1 PM are generally returned the same day. Remember that our office is open Monday through Thursday only. Calls are returned in the order they are received. If you call and leave numerous messages, your chart is pulled out of the original order while attaching a new message to it.  This delays a response to you and puts you at the bottom of the stack. 

HOW DO I GET PRESCRIPTION REFILLS?

New prescriptions require an appointment. Please request these during your appointment time if they are needed. If your insurance has changed and you need new mail-order prescriptions, this also requires an appointment - even if you were recently seen. Please always check with your pharmacy for refills. If there is a question, they will contact us electronically. Remember that our office is an Endocrine office only and requests for medications that do not pertain to your endocrine issue need to go through your Primary Care Physician (PCP).

HOW ARE REFERRALS HANDLED?

If your insurance plan is one that requires you to have a referral from your PCP, it is your responsibility to obtain your referral at least 1 week prior to your appointment. Your appointment will be canceled and rescheduled only when a valid referral is obtained. This is the policy of your insurance company, and we must adhere to it.

HOW ARE COPAYS/CO-INSURANCE HANDLED?

Our office accepts cash, check, credit or debit, and all co-pay/co-insurance is due at the time of service. Effective October 1st, all AHCCCS patients will be responsible for a $4-$5 co-pay as well. We are sorry, but no exceptions can be made to this policy. If you do not have your co-pay or co-insurance, please reschedule your appointment for a time when you do. Direct any questions you may have on this issue to your insurance company. 

HOW ARE LABS/LAB SLIPS HANDLED?

If you have lost/misplaced your lab slip or did not have your labs done for you at your scheduled appointment, please keep your scheduled appointment to have new paperwork done for you. If you have had lab work done and have a scheduled appointment, your results will be given to you at that time.

WHAT IF I AM A NO SHOW OR HAVE A LATE CANCELLATION?

If you can't make your scheduled appointment, please contact our office at least 24 hours in advance so that we may offer this appointment to another patient. There is a $35 no show/late cancellation fee that is charged if we do not fill your time slot. This fee must be paid before another appointment will be scheduled for you. Patients that have "no-showed" our office 2 times will be referred back to their PCP and will no longer be scheduled with our office.

WHAT IF I HAVE QUESTIONS?

If you a simple question, we are happy to answer it over the phone or over the Internet. If you have numerous questions or issues, please schedule an appointment to discuss personally. 

HOW DO I PROVIDE YOU WITH FORMS OR PAPERWORK?

If you have forms or paperwork that need to be completed and signed, make an appointment and it will be done at that time. If our office receives form completion requests for you, it will be placed in your chart until your next scheduled appointment.

HOW DO I OBTAIN MY MEDICAL RECORDS FROM MY PRIMARY CARE PHYSICIAN?
Here is a link to a form so you can have your medical records sent to our office. You will need to send it to your physician.
WHAT SHOULD I BRING TO MY VISIT?
Because we are a small medical practice, we are able to provide personalized service.  However, this means we need our patients to help us maintain certain policies. First, we respect that your time is valuable and we need our patients to respect that our time is also valuable.  We try to maintain a schedule and we need our patients to arrive on time. Second, we accept debit, credit, checks and cash at the time of service. We try to avoid sending bills to our patients, which helps us to focus on your health, not our finances. We encourage all patients to bring all their medications or an accurate list of their medications to each appointment. Please bring your home glucose monitor and your log book to all visits. Also, please know where you plan to have your prescriptions filled and know the pharmacy's phone number so we can email your prescriptions to your pharmacy for your convenience.
