Your First Visit

WHAT SHOULD I BRING TO MY FIRST VISIT?

Because we are a small medical practice, we are able to provide personalized service. However, this means we need our patients to help us maintain certain policies. First, we respect that your time is valuable and we need our patients to respect that our time is also valuable. We try to maintain a schedule and we need our patients to arrive on time.

We encourage all patients to bring all their medications or an accurate list of their medications to each appointment. Please bring your home glucose monitor and your log book to all visits. Also, please know where you plan to have your prescriptions filled and know the pharmacy's phone number so we can email your prescriptions to your pharmacy for your convenience.

History and physical forms will be completed at your first visit. Here are the links. You may fill out this form and bring it to your first visit.
